Provide Home Health Nurse Aide services in the homes of sick or disabled persons. Record information in patient’s medical records. Perform light cleaning duties, prepare simple meals and run short errands. Assist patients with baths and proper skin care. Observe skin for any disorders. Assist clients in getting dressed. Straighten bathroom after bathing client. Maintain chart on client regarding what was done for patient. Report any change in clients health/habits. Register vital signs of patient (weight, blood pressure, pulse, respiration and temperature as instructed by RN. Check vehicle for gas and oil and may assist with washing vehicle. May perform restorative services as instructed by Physical Therapist Consultant and RN if qualified report progress of patient to RN. May deliver supply list to hospital and may pick up necessary supplies. May shampoo, dry and comb/brush client’s hair if ordered. May run short errands for client if approved by supervisor. May make beds and prepare simple meals for client. May monitor medication compliance when directed by RN. May give enemas; insert suppositories when authorized by RN. Perform motion exercises or encourage mobility and exercise within medically determined limitation. Provide patient and family members with psychological and emotional support. Encourage realistic independence. Promote patient’s mental alertness through conversation and active listening. May provide assistance to nurses in mid-level or physician clinics within the scope of home health aide practice. May perform duties in mid-level or physician clinic assistance as assigned by the nurse.
